California-based medtech firm Semler Scientific has unveiled an ambitious Bitcoin acquisition strategy that could make it one of the largest corporate holders of the digital asset in the world. On June 19, the company announced plans to accumulate 105,000 BTC by the end of 2027, marking a significant escalation of its treasury policy.
This updated Bitcoin strategy includes interim targets of 10,000 BTC by the end of 2025 and 42,000 BTC by the end of 2026. The expansion will be funded through a combination of equity and debt financing, as well as operational cash flow.
A New Bitcoin Treasury Chief: Joe Burnett
To spearhead this effort, Semler has appointed Joe Burnett as its new Director of Bitcoin Strategy. Burnett is a well-known Bitcoin advocate and previously served as the Market Research Director at Unchained, a firm specializing in secure custody and financial services for BTC holders.
“Bitcoin is the ultimate long-duration asset,” Burnett said in a statement. “We’re building one of the largest corporate Bitcoin treasuries in the world not for short-term speculation, but for long-term security and value preservation.”
A Rapidly Growing BTC Portfolio
Semler first adopted Bitcoin as its primary treasury reserve asset in May 2024. Since then, the company has rapidly accumulated 3,808 BTC as of mid-May 2025, placing it 13th among publicly listed companies globally, according to BitBo data.
Recent acquisitions include:
- 894 BTC in Q1 2025 for $90.7 million
- 165 BTC on April 30 for $15.7 million
- 167 BTC a week later for $16.2 million
These purchases brought Semler’s total Bitcoin cost basis to approximately $340 million, with a fair market value of $387.9 million by early Junreflecting an unrealized gain of $177 million and an impressive 287% yield since inception.
Despite Healthcare Losses, Bitcoin Delivers Gains
While Semler continues to face operating losses in its core healthcare business, its Bitcoin strategy has driven overall positive financial outcomes. The firm reported a 22.2% BTC yield in the first quarter of 2025 and currently boasts one of the highest Bitcoin-per-share ratios among U.S.-listed companies, at 0.00034 BTC per share.
Part of a Broader Corporate Bitcoin Movement
Semler’s aggressive BTC expansion mirrors a broader trend among public companies turning to Bitcoin as a hedge against fiat debasement and a long-term store of value.
Tokyo-listed Metaplanet, for example, recently crossed 10,000 BTC in holdings six months ahead of its 2025 target. The firm now aims to hold 210,000 BTC by 2027, which would represent 1% of Bitcoin’s total fixed supply.
Meanwhile, financial heavyweights like BlackRock have continued accumulating Bitcoin through ETF-related purchases, contributing to institutional momentum and deepening liquidity.
Outlook
With its new roadmap, Semler Scientific is not only betting on Bitcoin it is staking its future identity on it. If the company achieves its 105,000 BTC goal, it would rival or surpass holdings of high-profile players like MicroStrategy and Tesla.
